National Guardian Life Insurance Company (NGL) has launched a new LTCi policyholder portal that empowers your clients submit claims, review documents, download forms and more.
Every policyholder received a letter with the following information: Policyholders now have the ability to:
- Select available correspondence and policy documents through the My Documents tab.
- Request and view annual statements and copies of their policy
- Review policyholder information, including policy details, premium details/history, beneficiary, and third-party designation through the My Policy tab.
- View claims information, including information on open/closed claims and claims payment history through the Claims tab.
- Submit various requests such as Claims Inquiry, Request for a Duplicate Policy, Beneficiary Change Form, etc., through the Contact Us section.
- Forms available under the Forms Center tab can be submitted through the Contact Us section which will be routed to the appropriate department.
- Submit claims that will be routed to Intake Specialists for review under Submit a Claim.
- Download or view the following claims and policy forms through the Forms Center tab:
- Claimant Statement (Form 208-2)
- Authorization (Form 329)
- PAC Change Form (NLTC-ACH-PAC)
- Policy Owner Change Form (NLTC-160)
- Third Party Change Form (NLTC-0148A)
- Direct Deposit Form (Form 3885)
- Beneficiary Change Form (Form NLTC 0140)
- Perform Login information updates
