PROGRAM OVERVIEW

Quafify for a Visa Gift Card Bonus for your Connecticut and New Jersey sales! You must submit LifeSecure long term care insurance applications for new business for the state(s) of Connecticut and/or New Jersey by November 15, 2014. Qualifying policies must be approved and issued by December 31, 2014. Your bonus payout will be based on policies approved and issued only.

 

5 – 9 issued policies = $250 Visa Gift Card

10 or more issued policies = $500 Visa Gift Card

 

Note: The amount payable will be determined based on the total number of policies issued during the qualifying period and is not calculated on a per policy basis.

 

OFFICIAL PROGRAM RULES

 

  • Qualifying policies must be submitted by November 15, 2014 and policies must be approved and issued by December 31, 2014.
  • Gift cards will be mailed after November 15, 2014 once qualifying policies are deemed approved and issued.
  • “Approved and issued” means approved by a LifeSecure underwriter and premium payment has been received by LifeSecure Insurance Company for the respective approved policy. Multi-life policies approved and pending payment via an employer list-bill process will also qualify.
  • Agent must be appointed with LifeSecure Insurance Company and in good standing at time the bonus is payable. Agencies do not qualify for bonuses.
  • Case splits will be treated and applied on pro rata basis per participating agent.

The bonus payout amount is subject to tax according to IRS regulations. Qualifying agents with a bonus payout will receive a 1099 form from LifeSecure Insurance Company.
