Revised Couples Form in Florida Required February 17, 2015

Beginning February 17, 2015, a revised Couples Form will be required for the
Total Living Coverage® product in Florida. This revised form will replace the form
previously included in the Required Point of Sale Forms packet.
The updated Required Point of Sale Forms packet for Florida will have a revision
date of February 1, 2015. The packet will be made available on producer
websites and normal fulfillment channels on February 17, 2015.
Important Dates and Transition Period:
Feb 1: Revision Date on updated Required Point of Sale Forms packet
Feb 17: Forms packet available on producer websites
Feb 17: Effective Date for revised Couples Form
Mar 20: Last Home Office Receipt Date for older versions of the Couples Form
