ATTENTION CALIFORNIA AGENTS: Ordering Printed LTC Applications & Forms

 

Effective Aug. 1, LTC Application and Regulatory Forms for California will no longer be bound into booklets. Instead, Application and Regulatory Forms will be ordered separately, as is done in other states.

 

Please note: There are no changes to the California applications or forms. Agents may continue using booklets until their supplies run out.

 

Why not choose our “green” option and deliver these documents to your clients via email?

 

Links to Zip folders containing PDFs of the Multi-life or Individual applications and forms are available in your Agent Portal. These links can be emailed instantly to your clients, an easy way to save both time and resources. Simply click on the “Resources” tab in your portal and select “Download Materials from PDF Library.” Then select “California” from the drop-down menu to copy/paste the appropriate links into an email. It’s that easy!
