Custom Care III featuring Benefit Builder to be discontinued in six additional states and D.C. on August 24, 2015.
Custom Care III featuring Benefit Builder in AZ, CT, DC, DE, FL, IN, and SD will be discontinued as of August 24th. Therefore, all Custom Care III featuring Benefit Builder applications must be signed and received in the Home Office no later than August 24th. LTC Captivate cases must be signed and submitted through the system no later than August 24, 2015.
