Custom Care III featuring Benefit Builder in New Jersey will be discontinued as of December 14^th. Therefore, all Custom Care III featuring Benefit Builder applications must be signed and received in the Home Office no later than December 14^th. LTC Captivate cases must be signed and submitted through the system no later than December 14^th.
