New Jersey no longer requires its state specific suitability questionnaire. Please stop using New Jersey’s Consumer Suitability and Due Diligence Questionnaire I-22733 (NJ). This form is no longer available and will no longer be accepted after March 31, 2016.
Instead, use the standard, non-state specific Consumer Suitability and Due Diligence Questionnaire I-22733 for all life and annuity applications (except for term life insurance and products submitted through OneAmerica Securities).
