The Authorization to Release Underwriting information to Insurance Agent/Agencyform (31368) is now available May 1 for standalone long-term care (LTC) applications administered by illumifin.
Why it’s important:If a traditional standalone LTC application is declined or rated and the client has signed the form 31368, the underwriter can provide you with additional details about the reason for that decision. The form is only applicable if an application is declined or rated.
How it works: If you would like to get more information about a standalone LTC app that’s declined or rated:
- Print the form from by clicking here, or search for Form 31368 on iPipeline’s FormsPipe system.
- Ask the client to complete and sign the form. Wet signature is required, and DocuSign cannot be accepted. Do not alter the form.
- Scan the completed form so you have a copy of it.
- Give the client the completed form.
- If submitting an eApp, upload the form along with any prescreen info as additional documentation (see image below).
- If submitting a paper application, submit the form with all other required forms.
