Special Forms are Required in Delaware, Georgia and Illinois
New Civil Union Notice/Domestic Partner forms may be required in DE, GA and IL beginning March 28, 2012. Please review the following to determine if one of these forms must be completed by or distributed to your client.
Any resident of Delaware, Georgia or Illinois applying for coverage in his/her home state (including single, married or civil union applicants).
DE: If a resident of Delaware is applying for a Delaware policy, then the agent must deliver the Civil Union Notice, form TLC CU-NOT DE 0212. This form must be delivered to all applicants, even if the applicant is not a civil union partner.
IL : If a resident of Illinois is applying for an Illinois policy, the agent must deliver the Civil Union Notice, form TLC CU-NOT IL 0212, even if the applicant is not a civil union partner.
GA: If a domestic partner is a resident of Georgia and is applying for a Georgia policy, then they must complete the Declaration of Domestic Partnership, form TLC 1-DDP (GA) 0102.
Domestic Partner/Civil Union Partner rules will only apply if the applicant is a resident in one of these 3 states in which they are applying for coverage. If a person is applying for coverage in these 3 states and is a resident of another state, or if a resident in these 3 states is applying for coverage in a different state, the Domestic Partner/Civil Union Partner rules do not apply to that individual.
Availability of forms and cutoff date for old forms
These new forms will be available in the application packages at the Agent Resource Center on March 21, 2012. The cutoff for use of the old application packages is April 18, 2012.
New 2012 Massachusetts Bulletin for People with Medicare
Beginning April 1, 2012 agents/producers must provide the 2012 version of the Massachusetts Bulletin for People with Medicare along with the Medicare Guide to Massachusetts resident applicants over 65 years of age. A 2012 version of the Massachusetts Bulletin for People with Medicare form will be available for order at the Agent Resource Center on Monday, March 19, 2012. The previous 2011 version cannot be used after March 31, 2012.
