Performance LTC applications must be:
- Signed between March 1, 2016 and December 31, 2016
- Received by December 31, 2016
- Placed by February 28, 2017

Eligibility, Rules & Special Notes
- Bonus available in states where Performance LTC is approved for sale.
- Bonus is not available in states that have not launched Performance LTC, including CA, HI, MT, NY and ND.
- Subject to individual state restrictions. Void where prohibited by law, including all business written in DE, PA and in MI for business issued at age 65 or older.
- Internal and external replacements will be excluded in AL, IN, KY, NC, SD and WI.
- Bonus payment will be made beginning June 2016 and made every 90 days thereafter.
- Bonus is non-transferable or exchangeable for an alternative award.
- Available to eligible producers under select MGAs, National and Life Brokerage Accounts.
- Advisors affiliated with banks, broker/dealers and wire houses and other select distributors are not eligible to participate.
- Any taxes related to the bonus are the responsibility of the recipient. In accordance with Federal IRS regulations, awards will be reported as income on the qualifiers 2016 IRS Form 1099.
